Why does the encoder encode only the second and subsequent elements of a SEQUENCE OF value?

The most likely reason of such behavior is an incorrectly passed address of the unencoded PDU. An extra level of referencing is needed (missing &) by passing the address instead of the value of the PDU.

The same is true if there are incorrectly initialized components of the enclosing PDU missing an extra level of referencing. For example, an initialized decoded field of an instance of OpenType is incorrectly accessed as:

a->decoded = _v1;

instead of (correctly) accessed as:

a->decoded = &_v1;
